What to bring if you want to enroll:

• Social Security Number (or document numbers for legal immigrants)
• Employer and income information (pay stubs or W-2 forms)
• Policy numbers for any current health insurance plans covering members of you household
• Proof of Illinois Residency
• Household information (who lives with you)
